Key Responsibilities:
- Conduct assessments and implement care plans to support individuals and families.

- Maintain accurate and up-to-date records in accordance with professional and organizational standards.
- Participate in case conferences and reviews, contributing to the continuous improvement of service delivery.

Requirements:
- Qualified Senior Social Worker with relevant certifications and registrations.
- 2 years relevant experience
- Proven experience in a social work role, preferably within a healthcare setting.
- Strong assessment and care planning skills.
- Excellent communication and interpersonal abilities.
- Ability to work collaboratively in a multidisciplinary team.
